• 2x2 pictures (2)
• NBI / Police Clearance
• PRC (if Nurse)
• TESDA (if Caregiver)
• Long Folder
• Certificate of Graduation
• Attendance to (2) HHC Seminars (P500.00 each)
• In House Training SRF (2 Weeks w/out pay)
• Annual P.E.
• Flu Vaccine (P600.00)
